 These forms are for clients who are already scheduled for an appointment. Appointments must be made through the office. You may print these forms, fill them out, and bring them with you when you come in for counseling. These forms are necessary, and you will be asked to fill them out when you arrive for your appointment if you do not complete them ahead of time.
